General description

Cytokeratin 20 is a 46-kDa intermediate filament protein that reacts primarily with gastricand intestinal epithelium, urothelium, and Merkel cells. Anti-Cytokeratin 20 is usefulin the identification of specific types of these epithelial cells under normal, hyperplastic, and neoplastic conditions. Cytokeratin 20 has been detected in various adenocarcinomas, including those of the colon and stomach. Merkel cell carcinomas have shown reactivity. Incontrast, carcinomas of the breast are generally non-reactive.

In the liver, the immunostaining of cytokeratins (CK) 7 and 20 has been used to distinguish usual peripheral cholangiocarcinomas (CC) and colorectal carcinoma metastasis (CRM). However, other subtypes of CC are not infrequent and may be particularly difficult to distinguish

Metastatic adenocarcinoma in the liver with an unidentified primary tumor site is a common clinical problem. Pathologists often are asked to identify the primary tumor site. The histologic picture itself usually is not helpful, because the histology may be similar

Immunohistochemistry is a useful method in the differential diagnosis between pleural mesotheliomas and metastatic adenocarcinomas to the pleura. Cytokeratin (CK) 5-6 is one of the most specific mesothelioma-associated antibodies. Cytokeratin 20 and CK7 have been used successfully in studies determining
